The intent ladder: not all searches deserve your money

Every treatment your clinic offers gets searched thousands of ways, but those searches sit on very different rungs of a ladder:

The single most common mistake we see in clinic ad accounts is a campaign built on rung-1 keywords because they have the biggest search volumes. Volume is vanity. Intent pays the rent.

The rule of thumb: if you can’t imagine the searcher pulling out a credit card within 72 hours, don’t pay for the click. Answer it with content instead.

1. Treatment + location

Don’t hide from these because you’re not the cheapest — a price-curious searcher who lands on a clear page explaining what’s included often books with the clinic that felt most transparent, not the one that felt cheapest.

3. Problem-first keywords

Patients don’t always know the treatment name; they know the problem: “acne scar treatment Dubai”, “double chin removal”, “sweating treatment underarms”. These often cost less per click than treatment-name keywords because fewer clinics think to bid on them — while the intent is just as strong.

4. Competitor and brand-adjacent terms

Bidding on competitor names is legal and common, but expensive and low-converting — the searcher wanted them, not you. If your budget is limited, this is the first family to cut. Protect your own brand name instead: it’s cheap, and it stops competitors intercepting patients who were already yours.

Negative keywords: where clinic budgets quietly die

The keywords you block matter as much as the ones you buy.
